Electronic Signature
Instructions on how to create an electronic
signature:
Duplicate Diplomas
The University
of Southern Nevada (USN) issues only one diploma to each
student. Only under special circumstances will a diploma be
re-issued. The reissued diploma will be printed exactly as
the original except the current officers signatures will
appear. No name changes will be honored for previously issued
diplomas. If you have questions pertaining
to your order please call the Registrar’s Office at (702) 968-2029
or email us.
Please complete the Duplicate Diploma Order Form.
The diploma will be marked with ‘Duplicate Diploma’. Please
enclose a check or money order (no cash) to the address on the
form. Also enclose a clear copy of a government issued photo
ID. Upon receipt of the form and fee it will take 6-8 weeks
for arrival of your diploma.
Enrollment & Graduation
Verification
Complete this form if you require
confirmation of your enrollment at USN for loans, insurance or
employer purposes. Turnaround time is 3-4 days (except during peak
periods) and you will be contacted via email once your request is
ready to be picked up, faxed or mailed.
FERPA
Student Files: The Buckley Amendment, formerly known as the
Family Educational Rights and Privacy Act of 1974, establishes that
a post-secondary student has the right to inspect and review
his/her academic records and generally prohibits outside parties
from obtaining the information contained in these records without
the student’s written consent. However, a student may waive
the right to review certain confidential information, for example,
letters of recommendation placed in the student’s file.
Personal Counseling--Non-Academic
Issues
Students needing personal counseling services
about non-academic issues (e.g., grief counseling, alcohol,
substance abuse and mental health) should contact
the Registrar/Director of Student
Services. While these services are not provided by
the University, the Registrar/Director of
Student Services provides students with a
confidential venue to voice concerns, ask questions and Student
Services can work with the student to refer him/her to appropriate
resources for additional assistance.
Student Contact Update
Complete this form when you have a name, address or
emergency contact change. Name changes must be accompanied by legal
documentation that confirms the new name. Turnaround time is 3-4
days (except during peak periods) and you will be
contacted via email once your request is ready to be picked up,
faxed or mailed.
Title IV Student Drug
Information
The University of Southern Nevada is
committed to maintaining a drug-free environment in compliance with
applicable laws. The unlawful possession, use, distribution, sale
or manufacture of controlled substances is prohibited on the
University campus or any of its facilities. Violation of this
policy will result in the appropriate disciplinary action.
Transcript
Request
Transcripts may be requested in two
forms. Official bears the signature of the Registrar, is printed on
official transcript paper and costs $5.00 each. Unofficial does not
bear the signature of the Registrar, is printed on plain paper and
is free. Turnaround time is 3-4 days (except during peak
periods) and you will be contacted via email once your request is
ready to be picked up, faxed or mailed.
When applying for residencies, scholarships, boards,
etc. an official transcript is required.
Veteran’s
Administration
The University of Southern Nevada is approved to offer VA benefits
for the following programs and locations:
- Doctor of Pharmacy (Henderson, NV and South Jordan, UT)
- Bachelor of Science in Nursing - 18 month (Henderson, NV and
South Jordan, UT)
- Accelerated Bachelor of Science in Nursing - 14 month (Hartwell
Bldg at Warm Springs)
- Master of Business Administration (Henderson, NV and South
Jordan, UT)
If you have been approved for VA benefits and have a Certificate of
Eligibility which confirms your percentage and length of time of
benefits, please contact the Student Services Office at
702-968-5246 in Henderson and 801-878-1040 in South Jordan.
One of our staff will discuss the next steps and assist you with
determining how your benefits will apply towards your tuition at
USN.
