Transcript Request 
Transcripts may be requested in two forms. Official bears the signature of the Registrar, is printed on official transcript paper and costs $5.00 each. Unofficial does not bear the signature of the Registrar, is printed on plain paper and is free.

When applying for residencies, scholarships, boards, etc. an official transcript is required.

  • Grading procedures are all based on a 90 percentile passing score. Pharmacy grades on a Pass or No Pass scale. Nursing and MBA grade on an A or F scale.

Enrollment & Graduation Verification
Complete this form if you require confirmation of your enrollment at USN for loans, insurance or employer purposes. Turnaround time is 3-4 days and you will be contacted via email once your request is ready to be picked up, faxed or mailed.
